Incident Report Form
The Maricopa County Sheriff's Office only retains incident and accident reports authored by its employees.
If the incident occurred within the jurisdiction of a local police department, you must contact the appropriate Law Enforcement Agency for the report, otherwise your request WILL BE DENIED.
